You can use this policy to help claim reimbursement towards costly and essential dental treatment such as, check-ups, hygiene visits, fillings, crowns and dental implants (not available on all policies) up to the policy limits.

Please read the policy documents carefully before applying and please note that you will only be able to apply for dental cover in 2024 between 20 May and 16 June. To apply you must first have registered with Mylifestyle (see next steps below).

Cancellation
Please note you are unable to cancel this insurance until the next renewal period (20 May – 16 June 2024) , unless you leave Defence or there is a change in circumstances, a list of which can be found in your policy summary. As this is a corporate policy, your cover will automatically end if you leave Defence. Unum Dental may contact you with options to continue your cover as an individual using the email address you provided when you registered to use the online portal.

1 - Register with mylifestyle
To apply for dental insurance you must first register with the mylifestyle hub. To register for mylifestyle, visit the relevant link below and follow the registration instructions.
Please note that your payroll ID is your MOD staff number (e.g. 123456A). If you are new to MOD, it can take up to 8 weeks for your details to be recognised by the hub.
If you have already registered for mylifestyle, then you can sign in using your username and password.

2 - select level of cover
Select a level of cover from the range of plans available, which can be found on the slides from the recent webinar. You could also use this handy guide. Comparing your own dentist’s charges to the cover available should help you pick a suitable plan.
All plans cover 100% of NHS dental charges and up to the fixed amount listed on your benefit schedule.

3 - apply
Once you have logged in to mylifestyle, select "mywellbeing" at the top of the screen, and then click "find out more" underneath the Dental Insurance heading. This will take you through to check your eligibility and apply for Unum Dental insurance coverage, which you can only do between 20 May and 16 June.

4 - receive confirmation
Unum Dental will send a welcome email/letter to confirm cover shortly after the policy start date on 1 July.

5 - start claiming
Visit your dentist and start making claims for insured treatments (as listed on your policy schedule) from the day your cover starts.
There is no dental health check required to join, and you can submit claims quickly and easily using the online claims portal (registration required). Watch the video found here to learn more. To ensure your claim is processed as quickly as possible, please include details of your treatment, your dentist's details and proof of payment. Claims should be submitted within 12 months of the completion of your last treatment in any course. Unum Dental reserve the right not to pay any claims submitted after 12 months.
Worldwide cover: If you need treatment while abroad, get a receipt (in English) and submit your claim in the usual way. You will be reimbursed according to your benefit schedule, using the exchange rate that applied on the date treatment was paid for.
